Hartford HealthCare and Cadence Announce Partnership to Expand Remote Care for Seniors with Chronic Conditions
Announcement of strategic partnership with Cadence to deploy Cadence’s Clinical Intelligence as part of Hartford HealthCare Remote Care, designed a new program to support older adults living with chronic conditions and extend preventive primary care beyond the traditional clinical setting.

With this collaboration, Hartford HealthCare with AI role support vital monitoring, proactive clinical support, and personalised lifestyle coaching into patients' homes, which helps individuals.

Executive Vice President and Chief Clinical Officer at Hartford HealthCare, Dr. Ajay Kumar, said, the company continues to innovate in how we deliver care bringing it closer to patients, identifying emerging concerns earlier, and intervening before conditions escalate.

According to Towards Healthcare, the U.S. home healthcare market is projected to experience significant growth, with estimates suggesting the market size will increase from USD 61.48 billion in 2026 to approximately USD 122.91 billion by 2035, representing a compound annual growth rate (CAGR) of 8% from 2026 to 2035. Growth is driven by the rising prevalence of conditions like diabetes, obesity, and cardiovascular disorders requires long-term patient monitoring. With technological advancement, rapid adoption of telehealth, remote patient monitoring (RPM), and wearable medical devices are revolutionizing at-home patient tracking.
